Medical Records Clerk

Position Summary

Responsible for accurately accounting for and facilitating completion of all medical records information. To serve as onsite liaison between the corporate office and the hospital to ensure that the needs of both are met.

Primary Duties and Responsibilities:

  • 40%: Ensure providers have completed all records upon end of shift or before/during next shift worked. Incomplete charts should be maintained at 1% or less
  • 15%: On a daily basis, send any necessary patient documentation to APP corporate office via SFTP (i.e. addendums, face sheets, code blue, etc.)
  • 15%: Ensures Provider’s orders and other records requiring signature are documented within the required time frame
  • 10%: Support Medical Director(s) for ED and/or HM services with any administrative needs
  • 10%: Interface with hospital HIM, IT, and other data departments to assist APP with ongoing needs
  • 10%: All other duties as assigned

Knowledge and Experience:

  • High school diploma with a minimum 1 year of related experience required
  • Medical terminology education preferred

Skills Required:

  • Basic computer and keyboarding skills required
  • Strong verbal and written communication skills required
  • Basic Word and Excel computer application skills. Proficient with Windows operating system and Microsoft Outlook program

Abilities Required:

  • Must be professional, prompt, and polite, with a can-do attitude toward problem solving

Physical Demands:

  • Minimal travel may be required
  • Continuously sitting
  • Frequent walking, standing, bending, kneeling, and reaching
  • Continuous repetitive use of arms, hands and fingers
  • Occasional lifting and carrying up to 20 pounds

Mental Demands:

  • High level of mental awareness
  • High level of attention to detail
